DIAGNOSTIC VALUE OF PRESEPSIN AND INTERLEUKIN-33 IN BACTERIAL INFECTIONS IN TERM NEONATES

Abstract:
This study evaluated the diagnostic value of presepsin and interleukin-33 (IL-33) in term neonates with bacterial infections. The prospective study included 36 term neonates with neonatal pneumonia or sepsis and 13 healthy controls. Serum C-reactive protein, procalcitonin, presepsin, IL-33, and lactate were assessed. Infected neonates had significantly higher concentrations of all biomarkers than controls (p<0.05), with the greatest increases observed for presepsin and IL-33. ROC analysis demonstrated excellent diagnostic performance for presepsin (AUC=0.971), CRP (AUC=0.971), lactate (AUC=0.965), and IL-33 (AUC=0.961), whereas procalcitonin showed lower accuracy (AUC=0.789). Presepsin and IL-33 demonstrated 94.1 % sensitivity and 100 % specificity. A strong positive correlation was found between presepsin and IL-33. These findings indicate that presepsin and IL-33 are highly informative biomarkers whose combined assessment with conventional laboratory markers may improve early diagnosis and clinical management of bacterial infections in term neonates.
