ALGORITHM FOR MANAGEMENT OF PULMONOLOGICAL PATIENTS IN AMBULATORY SETTINGS

Annotation | The organization and delivery of quality, high-quality medical care in ambulatory settings, including dynamic observation, examination, treatment and patient management in general is a daunting task for physicians of all specialties. This requires improvement of existing methods and finding new approaches to solving problems. The study was based on analysis of case histories of 1077 pulmonological patients (593 women and 484 men) who were on the observation from 2004 to 2011. Age 47.80 (33.95; 69.72) years. The method includes filling algorithm specially designed card with a description of patient's clinical and basic anamnestic data, creation and management of individual folders with an electronic archive of the results of clinical and laboratory research in the dynamics, an extended survey and setting a timetable for visits. Complexity of work of the pulmonologist confirmed by a variety of diagnoses (approximately 43) who have diagnosed during admission consultation, as well as the need for an analytical assessment of a large amount of information for a limited time. The effectiveness of the proposed algorithm for management of pulmonological patients displayed statistically significant (p <0,0001) decrease in the number of visits for exacerbation compared with the frequency of planned visits. Исследование выполнено на основе анализа историй болезни 1077 пациентов пульмонологического профиля (593 женщины и 484 мужчин), которые находились на наблюдении с 2004 по 2011 годы. Возраст 47,80 (33,95; 69,72) лет. Методика алгоритма включала заполнение специально разработанной карты больного с описанием основных клинико-анамнестических данных, ведения индивидуальной папки с электронным архивом результатов клинико-лабораторных исследований в динамике, расширен-ного обследования и установления графика визитов. Сложность работы врача-пульмонолога подтверждается разнообразием диагнозов (около 43), которые приходится диагностировать во время консультативного приема, а также необходимостью аналитической оценки большого объема информации в течение ограниченного времени. Эффек-тивность применения предложенного алгоритма ведения пульмонологического больного отображается в статисти-чески значимом (p <0,0001) уменьшении количества визитов по поводу обострений по сравнению с частотой плановых посещений. |
