    FEATURES OF SURGICAL TREATMENT OF PATIENTS WITH CSF LEAK FROM THE SPHENOID SINUS

    Annotation Relevance. Recently, due to the development of endoscopic surgery of the sellar-chiasmatic area, the surgical treatment of csf leak caused by a bone defect in the sphenoid sinus, seems to be an urgent and very significant problem. Objective. To evaluate the effectiveness of surgical treatment using various reconstruction techniques in patients with csf leak, the source of which is a bone defect in the sphenoid sinus. Materials and Methods. In 39 patients, the cause of csf leak was performed surgery for the removal of pituitary adenoma. The size of the defect ranged from 0.7 to 0.9 cm. In 21 patients, the reconstruction was made using the fascia lata, a fragment of autobone, followed by the use of a mucous flap on the feeding vascular pedicle. In 18 patients, a fragment of the fascia lata with a mucous flap was used. In 8 patients, the bone defect was located in the lateral recess of the sphenoid sinus. In 6 patients, the reconstruction was performed with a fascia lata, autobone, and a mucous flap. Two patients underwent obturation of the sphenoid sinus with auto-fat tissue. Conclusions. The reconstruction of bone defects in the area of the sphenoid sinus, the dimensions of which exceed 0.7 cm should be carried out using a fragment of the fascia lata, which is fixed in the bone window by autobone like the “latch” type.
